When you come for your appointments, please bring the following:

• Driver’s License or a valid ID

• Insurance information

• Referral Authorization or letter (if required)

• Discs and reports, X-rays, MRIs, CT scans, etc. and any other relevant information

• Images on discs are preferred

• List of medications (if any)

• Co-payment (if needed). Payment is accepted by cash, check, credit or debit card.

• A completed patient registration and health history form
